Creates and updates assisted living resident service plans according to resident level of care and needs. Reviews physician orders and monitors medication administration. The LPN follows up on all AL resident incident reports and completes admission assessments. Assists caregiver's in providing services to frail, ill and memory challenged residents.
Completes preadmission assessments, creates and updates assisted living resident service plans. Reviews physician orders and maintains the resident medication administration master record using an electronic health record. The LPN follows up on all AL resident incident reports and provides statistics for the Risk Management committee. The LPN assists and trains caregiver's to ensure resident's service plan is carried out. The LPN may set up medication boxes.
Graduate of an approved school for licensed practical nursing. Must hold a current, valid license in the State of Arizona. Previous experience in a geriatric setting preferred. Must be flexible and have a desire to communicate and work with seniors. Must have 2 years Assisted Living experience. Knowledge of Assisted Living rules and regulations. Must be motivated, self-starter, with a passion to serve seniors.
