  • Overview

    The facility’s Resident Assessment Instrument (RAI) process is coordinated by a Registered Nurse (RN) in accordance with state and federal regulations. This position is responsible for ensuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines regarding the Minimum Data Set (MDS) / RUGS process. An RN services as the Minimum Data Set Department Manager. The MDS/RAI Director facilitates the completion of Omnibus Reconciliation Act (OMBRA) and Prospective Payment System (PPS) assessments. This position also includes direct supervision of the facility MDS Coordinators and the QA Nurse.

    Responsibilities

    1. Works well with all members of the Interdisciplinary Team and promote an interdisciplinary team approach to the Care Plan process at all times.
    2. Establishes individual MDS schedules and coordinates assessment activities to all the clinical disciplines for each resident maintaining regulatory compliance.
    3. Ensures MDS’ are encoded accurately, edited, locked in, and electronically transmitted within the required time constraints.
    4. Determines the RUG’s category of Medicare payments for residents based on the MDS assessments with the collaboration of the Interdisciplinary Team.
    5. Performs a nursing physical and psychosocial MDS assessment on all residents, AIMS or DISCUS tests on appropriate residents quarterly, and reassessments as per the MDS schedule requirements, unless these are assigned to other clinical disciplines for completion.
    6. Demonstrates the ability to organize and implement MDS and Care Plan schedules.

    Qualifications

    1. Graduate of an approved school for RN’s with a current active license in good standing in the state of Arizona.
    2. AANAC RAC Certification
    3. Current CPR and valid DPS Fingerprint Card.
    4. Two years of management and MDS Coordinator experience within a long-term care setting.
    5. Must be able to work effectively with a nursing team and be resourceful and level-headed in emergency situations.
    6. Must treat resident information with confidentiality at all times.
    7. Must exhibit understanding, patience, and tact when dealing with residents, family members, visitors, and staff.
    8. Must be detail-oriented and work well under the stress of deadlines.
    9. Must be willing to perform some non-job-related tasks/duties.
