    With campuses in Phoenix, Mesa, Historic Mesa, Tucson, Cottonwood, and Surprise, Christian Care is the largest provider of non-profit senior housing and healthcare services in Arizona.

     

    We are currently seeking a full-time LPN/Assisted Living Supervisor to join our team at Fellowship Square Historic Mesa. The general schedule for this role will be Tuesday thru Saturday, from 7 am to 3:30 pm.  This position must be flexible, however, to accommodate for the needs of residents and their families, to provide assistance to Caregivers, and maximize training/educational opportunities.

    Responsibilities

    • Serve as the Oasis Nurse and Medical Supervisor, including training the Caregiver staff for hands on work and related equipment
    • Review and sign the resident service plans, MARS and TARS. Perform monthly review of MAR and ADL records, and additional audits as needed for QA purposes.
    • Perform medication pass daily as instructed by ALM. This may include morning and/or afternoon shifts.
    • Administer TB testing for IL & AL Residents, Staff, and Job Candidates. This includes monthly campus TB clinics.
    • Responsible for scheduling Caregivers, contacting agencies for support when needed, and assisting ALM with bi-weekly payroll processing.
    • Provide daily supervision and direction to Caregiver staff, including ongoing coaching and employee development. Complete both 6 month and 1 year performance evaluations.
    • Interview Caregiver candidates, contact references, and obtain documentation based on the requirements of the role.
    • Maintain good working relationships with visiting doctors, hospice agencies, pharmacies and other related vendors.

    Qualifications

    • Must be a LPN
    • Demonstrated ability to make independent decisions when circumstance warrants such action and remain calm during emergency situations.
    • Communicate tactfully with personnel, residents, visitors and the general public.
    • Proficient with Microsoft Outlook, Word, and Excel.
    • Experience with electronic medical records preferred.
    • Ability to speak, read, write, and understand English fluently.
    • Pre-employment drug screen and background check.
