MDS Coordinator (RN) for a 68-bed skilled nursing facility. Full time day shift.

Establishes individual MDS schedules and coordinates assessment activities to all the clinical disciplines for each resident maintaining regulatory complianEnsures MDS’ are encoded accurately, edited, locked in, and electronically transmitted within the required time constraints. Determines the RUG’s category of Medicare payments for residents based on the MDS assessments with the collaboration of the Interdisciplinary Team. Performs a nursing physical and psychosocial MDS assessment on all residents. Demonstrates the ability to organize and implement MDS and Care Plan schedules. Demonstrates competence in computer, analytical, and problem-solving skills. Responsible for transmission of MDS to Arizona D.H.S. Responsible for complete assessment of residents’ needs, initiate and complete MDS & RAP modules per regulation standards. Maintains quarterly, annual, and change of condition revisions necessary, and updates the resident plan of care quarterly, and from MDS revisions.Promotes positive, effective, and collaborative working relationships with all departments to ensure compliance with all R.A.I. and care plan requirements. Participates in training programs for nursing personnel re: MDS process, Care planning, etc. Assures the facility’s compliance with the provisions of the Medicare program.Performs other job-related duties as assigned by the Director of Nursing. On a rare occasion, may be asked to perform Staff Nurse duties.
Must be currently licensed in the State of Arizona as a registered nurse and must show evidence of current, valid license. Experience in a long-term care setting required. Experience in MDS 3.0 is required. Good communications skills necessary including fluent English, both verbally and written. Must have strong working knowledge of PDPM and minimum of 3 years MDS experience in long term care setting.
