Overview

The purpose of the Physical Therapist Assistant position is to assist the Physical Therapist in providing skilled Physical therapy services to patients of Covenant Home Health Agency.  The Physical therapist assistant is required to perform his/her duties in accordance with the current applicable Federal, State and local standards, guidelines and regulations and Covenant established policies and procedures.  

 

The Physical Therapist Assistant is a Covenant Home Health employee.  The Director of Nursing is responsible for recruiting, hiring, evaluating, and supervising Physical therapists to provide services for Covenant

HHA.  The HHA DON provides on-site supervision and communicates with the Administrator regarding all personnel issues regarding the Physical therapist. 

 

The Physical Therapist Assistance will function in accordance with the state of Arizona professional standards, the state’s physical therapist practices act which can be found at the HHA or Human Resources.  The therapist will also follow Covenant Home Health policies and procedures covering the scope of services offered by the physical therapy department. 

Responsibilities

  • Review physician’s referral and patient’s medical record to ensure compliance with the recommended physical therapy program required.
  • Ensure each patients approved evaluation techniques, tests and observations, and prepare written treatment programs based on evaluations.
  • Follow the prescribed physical therapy program to restore and maintain maximum function and prevent disability using a range of therapeutic modalities.
  • Coordinate physical therapy services with occupational therapy, speech‐language pathology and the nursing department.
  • Maintain appropriate documentation in the patient chart.
  • Communicate physical therapy philosophy and treatment approaches and goals to patients, families and other members of the rehabilitation team.
  • Prepare and submit timely clinical documentation for billing.
  • Adhere to Agency mission, Code of Conduct, policies and procedures, and all state and federal regulations.
  • Maintain productivity standards.
  • Full utilization of technology provided to document and enhance client care.
  • Any and all other duties as assigned.

Qualifications

  • A valid registered license for Physical Therapist Assistant the state in which employed.
  • Acute care, long‐term care and/or rehabilitation experience preferred.
  • Excellent observation, verbal and written communication skills, problem solving skills, and basic math skills.
  • Effective interpersonal skills with strong focus on customer service.
