Overview

The purpose of the Occupational therapist position is to provide skilled Occupational therapy services to patients of Covenant Home Health Agency. The Occupational therapist is required to perform his/her duties in accordance with the current applicable Federal, State and local standards, guidelines and regulations and Covenant established policies and procedures.

The Occupational therapist is a Covenant Home Health employee. The Director of Nursing is responsible for recruiting, hiring, evaluating, and supervising Occupational therapists to provide services for Covenant HHA. The HHA DON provides on-site supervision and communicates with the Administrator regarding all personnel issues regarding the Occupational therapist.

The Occupational therapist will function in accordance with the state of Arizona professional standards, the state’s Occupational therapist practices act which can be found at the HHA or Human Resources. The therapist will also follow Covenant Home Health policies and procedures covering the scope of services offered by the Occupational therapy department.

Responsibilities

• Review and evaluate physician’s referral and patient’s medical record to determine
occupational therapy program required.
• Evaluate each patient’s current level of functioning using approved evaluation
techniques, tests and observations, and prepare written treatment programs based on
evaluations.
• Administer a prescribed occupational therapy program to restore and maintain maximum
function and prevent disability using a range of therapeutic modalities specific to upper extremity function, sensory, dressing, transfers, eating, and homemaking.
• Coordinate therapy services with physical therapy, speech‐language
pathology and the nursing department.
• Maintain appropriate documentation in the patient chart, and re‐evaluate patient
progress and revise treatment programs when necessary.
• Communicate the therapy philosophy and treatment approaches and goals to
patients, families and other members of the rehabilitation team.
• Supervise Rehab COTAs, monitor productivity and staffing needs within the agency.
• Prepare and submit timely clinical documentation for billing.
• Adhere to Agency mission, Code of Conduct, policies and procedures, and all
state and federal regulations.
• Maintain productivity standards.
• Full utilization of technology provided to document and enhance client care.
• Any and all other duties as assigned.

Qualifications

• Graduate of an accredited school for occupational therapy and a valid registered license for
the state in which employed.
• Acute care, long‐term care and/or rehabilitation experience preferred.
• Excellent observation, verbal and written communication skills, problem solving skills,
and basic math skills.
• Effective interpersonal skills with strong focus on customer service.
