Job Description: 

RN needed with Home Health experience preferred, knowledgeable of Medicare guidelines, JC and all OASIS forms. Minimum travel - most visits are on our Fellowship Square campus.

Assumes responsibility for the overall supervision of nursing care relating to both care of the residents and staffing, in cooperation with other members of the health care team. Provide medication setups for residents one day per week.

Must be currently licensed in the State of Arizona as a registered nurse and must show evidence of current, valid license. Experience in a long-term care setting required. Prior experience in a charge position preferred. Good communications skills necessary including fluent English, both verbally and written. Follows/adheres to organization, Federal/State regulations and guidelines, including Resident's Rights. Maintains neat, well- groomed appearance, adhering to dress code of organization/program.
