Our Christian Care Fellowship Square -Nursing Center is seeking an experienced and passionate Social Services Director professional to join our team. Job Description:

Looking for an exsperienced Socail Service Director for short-stay rehabilitation facility with low cencus. A background in Activities is a plus.

Assists in the identification of social, psychological, and emotional strengths and needs of residents. Services to include placement within the facility or home as required under State and Federal regulations and policies set forth by Christian Care Health Center.

• Conduct interviews with family members and, when possible, residents to determine assessment and needs prior to admission.
• Schedule admission to the facility based on patient needs and availability.
• Develop social history summaries to study, identify and provide for the social, emotional, and psychological needs of the resident.
• Aids in residents’ adjustment to their medical and social condition and to our facility.
• Works with residents and/or families in the planning of future discharge alternatives.
• Encourages family contact and support of the resident.
• Provides intensive and/or continuing counseling to residents in the event of a crisis or need.
• Other duties as assigned.

• Bachelor’s degree in social work is preferred.
• Minimum of two years’ experience in a health care facility in Social Services.
• Must be able to articulate clearly and precisely.
• Must be able to deal effectively with stress created by sick residents, multiple tasks, noises, and interruptions, and work cooperatively as part of the health care team while maintaining a pleasant personality.
• Empathy and patience.
• Ability to build trust and long-term relationships with clients.
• Great interpersonal and communication skills
