Job Description:

Assist Fellowship Square residents with Social Service resources and maintain communications with residents and their family members to confirm residents can live safely in Independent Living. This position is also responsible for completing the Life Steps assessment with residents at their annual lease renewal.

Assists in the identification of social, psychological, and emotional strengths and needs of residents. Services to include placement within the facility or home as required under State and Federal regulations and policies set forth by Christian Care Health Center.

• Conduct interviews with family members and, when possible, residents to determine assessment and needs before admission.
• Schedule admission to the facility based on patient needs and availability.
• Develop social history summaries to study, identify, and provide for the social, emotional, and psychological needs of the resident.
• Aids in residents’ adjustment to their medical and social condition and our facility.
• Works with residents and/or families in the planning of future discharge alternatives.
• Encourages family contact and support of the resident.
• Provides intensive and/or continuing counseling to residents in the event of a crisis or need.
• Other duties as assigned.

• Bachelor’s degree in social work is preferred.
• Minimum of 2 years of experience in a health care facility in Social Services.
• Must be able to articulate clearly and precisely.
• Must be able to deal effectively with stress created by sick residents, multiple tasks, noises, and interruptions, and work cooperatively as part of the health care team while maintaining a pleasant personality.
