Fellowship Square-Nursing Skilled Center is seeking an experienced and dedicated Social Services Professional to join our team. If you have a passion for aiding individuals and are committed to elevating their life quality, this role is ideal for you.

In this position, your primary objective will be to support our patients in transitioning to the most appropriate level of care with appropriate resources on discharge while meeting all state and federal regulatory requirements.  

This short-stay rehabilitation facility has a low census, and experience in activities and admissions is a plus.

• Conduct interviews with family members and when possible, with residents to determine assessment and needs during the admission period.  
• Develop social history summaries within 14 days of admission to study, identify and provide for social, emotional and psychological needs of the resident to include trauma care assessments and care plans.
• To assist in residents’ adjustment to their medical and social condition and to our facility.
• Work with IDT and family to set up Care Plan meetings.
• Work with IDT as the overall Discharge Coordinator to assist the resident and/or family in planning of future discharge alternatives.
• Encourage family contact and support of the resident.
• To encourage family contact and support of the resident.
• Offer intensive and ongoing counseling to residents during times of crisis or when the need arises.
• Participate in reviewing residents' care plan, identifying needs, goals and approach and recording outcome and assessment in medical record at least quarterly.
• Maintain and increase awareness of community resources available for residents, families and/or staff. 
• Other duties as assigned.

• Bachelor's degree in Social Services work or relevant experience is preferred.
• Minimum of two (2) years’ experience in a health care facility in Social Services Department participating in social services activities and discharge planning.
• Must be able to articulate clearly and precisely.
• The ability to effectively manage stress caused by ill residents, numerous tasks, noise, and interruptions, and to work collaboratively as part of the healthcare team while preserving a pleasant demeanor is essential.
• Empathy and patience are essential.
• The ability to cultivate trust and maintain long-term relationships with clients.
• Exceptional interpersonal and communication skills.

Schedule: Monday through Friday and every other weekend will be on-call.
