Overview

Covenant Home Health Agency is seeking a dedicated and compassionate PRN Physical Therapist Assistant (PTA) to provide exceptional care to senior residents within the West Valley and Central Phoenix Senior Living Communities. Prior home care experience is required. 

 

This is a PRN (as-needed) position, offering flexible hours based on the needs of the patients and your availability. As a PRN, you will work under the supervision of a licensed Physical Therapist to assist in the implementation of patient care plans, improving the mobility, strength, and quality of life of our elderly clients.

Responsibilities

  • Review physician’s referral and patient’s medical record to ensure compliance with the recommended physical therapy program required.
  • Ensure each patient’s approved evaluation techniques, tests and observations are conducted and prepare written treatment programs based on evaluations.
  • Follow the prescribed physical therapy program to restore and maintain maximum function and prevent disability using a range of therapeutic modalities.
  • Coordinate physical therapy services with occupational therapy, speech‐language pathology and the nursing department.
  • Maintain appropriate documentation in the patient chart.
  • Communicate physical therapy philosophy and treatment approaches and goals to patients, families and other members of the rehabilitation team.
  • Prepare and submit timely clinical documentation for billing.
  • Adhere to Agency mission, Code of Conduct, policies and procedures, and all state and federal regulations.
  • Maintain productivity standards.
  • Full utilization of technology provided to document and enhance client care.

Qualifications

  • Licensed Physical Therapist Assistant (PTA) in the state of Arizona.
  • Current CPR certification.
  • Previous home health experience is required.
  • Excellent observation, verbal and written communication skills, problem solving skills, and basic math skills.
  • Ability to work independently and as part of a collaborative team
  • Compassionate, patient, and dedicated to improving the quality of life for senior patients.
  • Reliable transportation and the ability to travel within the West Valley and Central Phoenix areas.
