Overview

We are currently seeking a Home Health Registered Nurse/Case Manager (RN) to join our dedicated team.  In this role, you will provide care primarily within senior living campuses, ensuring residents receive exceptional, personalized care in a compassionate and supportive environment.

 

As a Registered Nurse/Case Manager, you will be responsible for assessing, coordinating, and managing the overall care of patients in their homes or senior living communities through Covenant Home Health, which is primarily operated by the parent organization Fellowship Square, Christian Care. This partnership reflects our shared mission and values of providing compassionate care and dedicated support to our residents.

 

Care is provided 90% of the time on senior living campuses, where you will collaborate closely with an interdisciplinary team to develop and implement care plans that ensure the individual needs of each patient are met.

 

Schedule: We are currently hiring Full-Time and PRN schedules.

 

Coverage needed at Senior Living Communities:

  • West and East Valley areas. 

Responsibilities

  • Case Management: Develop and implement individualized care plans for patients, coordinating with physicians, family members, and other healthcare providers to ensure optimal care.
  • Patient Assessment: Conduct thorough assessments of patients’ physical, emotional, and social needs and update care plans as required.
  • Care Coordination: Ensure the timely delivery of care services, including scheduling visits and coordinating with other healthcare professionals.
  • Medication Management: Oversee medication administration, monitor for side effects, and educate patients and families on proper medication use.
  • Patient Education: Provide education on health management, disease prevention, and improving quality of life for patients and their families.
  • Documentation: Maintain accurate and up-to-date records for all patient visits, including care plans, progress notes, and communication with the care team.
  • Advocacy: Act as an advocate for the patient’s needs, ensuring that care is delivered in accordance with the patient’s preferences and goals.
  • Collaboration: Work closely with physicians, therapists, and other care team members to ensure a coordinated approach to patient care.

Qualifications

  • Active RN (Registered Nurse) license in Arizona with current, valid credentials.
  • Two (2) years’ Registered Nursing experience in Home Health required.
  • Must pass background check and meet all health and safety requirements.
  • Valid driver’s license and reliable transportation required.
  • Strong assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently, manage time effectively, and provide compassionate care to seniors.
  • Ability to travel to designated service areas, including West Valley to Central Phoenix or Central Phoenix to East Valley to provide care to patients within their homes or Senior Living Communities.

Pay rate information: Our RN Case Managers are paid per visit, with compensation rates varying based on the type of service provided. There are seven different pay rates corresponding to specific visit types, ensuring competitive and fair compensation for each service.
