Overview

We are currently looking for a Licensed Practical Nurse (LPN) to join our Assisted Living and Memory Care team at Christian Care. In this role, you will provide skilled nursing care to seniors within our Senior Living Community.

 

The LPN will be responsible for delivering direct patient care, which includes administering medications, providing wound care, and assisting with daily activities, all under the supervision of the RN Case Manager or Director of Nursing.

Responsibilities

  • Prepare and update, as required, all resident service plans.
  • Perform assessments on prospective residents to determine the level of care for assisted living services or memory care.
  • Oversee the medication delivery plan for each resident, including self-administration and medication monitoring, medication ordering, physician orders, and recapping MARs to the physician’s orders.  
  • Perform a weekly narcotic count and maintain documentation of all narcotics provided to residents.
  • Schedule weekly showers and review caregiver documentation of ADLs.
  • Maintain resident injections such as tuberculosis skin testing; pneumonia vaccinations, Flu vaccinations, and B-12. 
  • Assist the assisted living manager with implementing and coordinating policies and procedures, and ADHS rules and regulations for our assisted living and Memory Support floor.
  • Assist the Assisted Living Manager with reviewing and revising policies and procedures that govern the operation of nursing and caregiving services provided in the assisted living facility.
  • Assist the Assisted Living Manager in the development and use of departmental policies and procedures, and establish a rapport in and between departments so each can see the importance of teamwork.
  • Interpret the facility's policies and procedures to employees, residents, family members, visitors, government agencies, etc., as necessary.
  • Assist the AL manager in the planning, conducting, and scheduling of in-service training classes for caregivers, including on-the-job training and orientation programs to ensure that current material and programs are continuously provided.
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the facility. 
  • Attend and participate in workshops, seminars, etc., to keep abreast of current changes in the long-term care field, as well as to maintain a professional licensure status. Assist the AL Manager in assuring the facility is maintained in a clean and safe manner for residents’ comfort and convenience by assuring that necessary equipment and supplies are maintained to perform such duties/services.
  • Assure that all facility personnel, residents, visitors, etc., follow established safety regulations, to include fire protection/prevention, smoking regulations, infection control, etc.
  • Maintain confidentiality of all resident information.
  • Assure that the residents’ rights to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights, including the right to wage complaints, are well established and always maintained.
  • Make routine inspections of the facility to ensure that established policies and procedures are implemented and followed.
  • Maintain an excellent working relationship with the medical profession and other health-related facilities.
  • Review accident/incident reports and participate in the campus risk management committee.
  • Assist the Assisted Living Manager and Assisted Living Department with maintaining compliance with AZ state rules and regulations.
  • Assist with the implementation and maintenance of the Comfort Matters programs
  • Assist AL manager with Risk and Quality Management ProgramsFollow up with residents’ conditions and caregivers’ concerns.
  • Assist the AL team with the health and wellness of all AL residents in Oasis and M2.
  • Answer calls from residents’ team members and families for emergencies after hours and on weekends.

Qualifications

  • Must, as a minimum, hold a Licensed Practical Nurse Certification acknowledged by the State of Arizona.
  • Must have, as a minimum, three (1) year of experience in an assisted living or long-term care environment. Additional consideration will be given for experience in memory care.
